Queensland has more than one form called Form 12, and they are not interchangeable. Opening the wrong one is a quiet mistake - the form looks plausible, the fields get filled, and the problem only surfaces when someone at council asks why a building certificate is attached to a plumbing permit application.

The plumbing and drainage Form 12 is titled "Compliance statement for specialist work". Business Queensland lists it with the plumbing and drainage forms, and the official PDF says it is used for section 45(2)(b) of the Plumbing and Drainage Regulation 2019.

How to stop the mix-up happening again
The fix is naming, not memory. Use "QLD PDR Form 12" or "Form 12 specialist work compliance statement" in your job software, your filenames, and your emails.
That one habit means the builder, the office, and the council are all talking about the same document. A file called form-12.pdf sitting in a job folder is a question waiting to be asked; form-12-pdr-specialist-work-[site].pdf is not.
Which law and form set applies?
The form title is the fastest check, but the legal reference settles it. The plumbing Form 12 PDF says it is used for section 45(2)(b) of the Plumbing and Drainage Regulation 2019. It belongs in the plumbing permit process and its declaration is about specialist work complying if it is carried out under the permit applied for.
The building Form 12 PDF says it is used under sections 74 and 77 of the Building Regulation 2021. It is inspection evidence for a component of a stage of assessable building work. The appointed competent person is certifying an aspect they inspected, not giving a plumbing specialist-work statement for a permit application.
If the job folder says only "Form 12", look at the governing approval. A plumbing permit application points to the PDR form. A building development approval and an aspect inspection request from a building certifier point to the building form.
What job event triggers each form?
The plumbing Form 12 looks forward. Its declaration states that the specialist work will comply with the code requirements if it is carried out in compliance with the permit applied for. That makes it part of the evidence used to decide the application, alongside the work description, technical basis and supporting documents.
The building Form 12 looks back at an inspected aspect. The appointed competent person completes it after inspecting the aspect and being satisfied it has been completed and complies with the building development approval. Business Queensland explains that the competent person gives it to the building certifier, who uses the inspection evidence when confirming the stage.
Those two moments should stop the mix-up. One supports approval for specialist plumbing or drainage work. The other reports an inspection of building work already completed.
Who signs and who receives each form?
The plumbing declaration is made by a suitably qualified person for the specialist work. The official form lists occupational plumbers or drainers, people the local government considers competent, relevant QBCC licensees, registered professional engineers and an "other" option. The statement is collected by local government for deciding the application and monitoring compliance.
The building certificate is signed by an appointed competent person. The building certifier must assess and appoint that person for the relevant inspection help before the certificate can be used. The competent person gives the signed Form 12 to the building certifier, not to the plumbing permit team as a substitute for the PDR statement.
The same individual might hold qualifications relevant to both workflows, but that does not make the forms interchangeable. Check the role they are performing on this job and who requested the document.
What evidence belongs with each form?
Both forms ask for the land, scope, technical basis and reference documents, which is why they look similar at a glance. The surrounding evidence is different.
Keep the plumbing statement with the permit application, current specialist design documents, specifications or calculations, local government reference and correspondence about the application. Keep the building certificate with the building development approval, the request to inspect, inspection notes, evidence for the aspect, building certifier reference and the stage inspection record. The building form asks for the building or structure class and the date the request to inspect was received, which are not the organising fields of the plumbing statement.
Filing each form with its own approval trail makes the choice visible months later. A loose PDF called form-12.pdf removes that context.
How should the office label the two forms?
Use the full workflow name in job software, filenames and emails:

Add the receiving role to the task as well. "Send PDR Form 12 with plumbing permit application" is clear. "Send building Form 12 to building certifier" is clear. "Finish Form 12" leaves the next person to reopen the job and work out which form was meant.
Saved business and licence details can still reduce retyping, but keep separate presets for the two form sets. Before download, preview the title, legal reference, signer role and receiver. Those four checks catch the wrong-form mistake before a plausible-looking PDF leaves the office.
Four quick job examples
A specialist hydraulic design supports a plumbing permit application. The designer or other suitably qualified person is stating that the proposed specialist work will comply if it is carried out under the permit applied for. That points to the PDR Form 12. Store it with the application, design documents and local government reference.
A waterproofing aspect has been inspected during building work. The building certifier appointed a competent person to inspect that aspect, and the person is satisfied it is complete and complies with the building development approval. That points to the building Form 12. Give it to the building certifier with the inspection evidence.
The builder asks for “Form 12” before the plumbing permit is issued. Do not choose from the number alone. Ask whether they need a specialist-work statement for the plumbing application or inspection evidence under the building approval. The answer decides the form set, signer, supporting documents and receiver.
The office receives a signed Form 12 with no useful filename. Check the full title and legal reference on the first page before filing it. A reference to the Plumbing and Drainage Regulation belongs with the plumbing application. A reference to sections 74 and 77 of the Building Regulation belongs with the building inspection record.
