Is this the right NSW Fire Safety Statement?

The NSW Fire Safety Statement (AFSS) records that fire safety measures in a building meet the required standard. NSW Planning publishes the official Version 4 template under Part 12 of the fire safety regulation.

Who has to complete the NSW Fire Safety Statement?
Building owners or their agents, with fire safety measures assessed by accredited practitioners (fire safety) and declarations signed by an authorised declarant.
When is it required?
Annually for buildings with fire safety measures on the schedule, or as a supplementary statement for critical measures between annual statements. Check current NSW timing rules before issuing.
Where does the completed form go?
Give copies to council, Fire and Rescue NSW (afss@fire.nsw.gov.au), and display a copy in the building foyer. Lodgement and display are your responsibility. Tradie Forms does not lodge the statement for you.
