QLD Form 9 Backflow Test Report
Help for completing Queensland Form 9 backflow test reports in Tradie Forms.
Form 9 is the approved form for registration and report on inspection and testing of testable backflow prevention devices under the Plumbing and Drainage Regulation 2019.

Before you start
- You need the site street address, owner or occupier contact details, and device test readings from the job.
- Copies of the completed form must go to the local government and the property owner within 10 business days after testing.
Sections in this guide
- Description of Land — site address and local government area
- Owner/Occupier Contact Details — name, postal address, phone, and email
- Test Criteria — protection, device type, and test type
- Device Location — where the device is and mains pressure
- Main Device — main backflow device test results
- By-Pass Device — bypass device when fitted
- Pressure Type Vacuum Breakers — PVB test results when applicable
- Air Gap — registered air gap or break tank measurements
- Test Kit Details — test kit serial and verification date
- Authorised Tester Details — your licence and contact details
- Contractor Licence — when you are not the responsible contractor
- Authorised Tester's Results — pass or fail and comments
- Declaration — signature and date
After export
