Custom forms

Request a new public template or a private template for your business

If the form you need is not in the public library, use Request a template.

You can request:

  • A shared template suggestion that may become public
  • A private custom template for your business

Custom templates can be compliance, daily operational, or customer-facing paperwork.

Request a template dialog with shared and private template options
Choose who the template is for, set the state and trade, then name the form. Scroll the dialog for Details, where you describe the use case and link a sample PDF.

Where to request a template

Use Request a template from:

  • Templates
  • New form when there is no good match
  • Support
  • Related template pages

You need to be signed in.

Request types

Final build pricing depends on the PDF, number of fields, signatures, tables, calculations, and special download needs. Your plan keeps the template available for your workspace.

What to include

Add:

  • State and trade
  • Template name
  • Why you need it
  • Who receives or lodges the finished PDF
  • Whether it should be public or private
  • A PDF sample if you have one

PDF attachments must be 10 MB or smaller.

What happens next

  1. Tradie Forms receives the request
  2. We review the PDF, scope, and pricing
  3. We follow up if anything is unclear
  4. Once ready and added to your workspace, the template appears in Templates

Private templates require a paid plan because they use cloud drafts and account access.

Custom templates are built by the Tradie Forms team. They do not appear instantly after request.
