This form prepares the ARCP PDF. It does not notify SafeWork NSW, replace the site-specific SWMS or confirm that the work can start.

Check the current SafeWork NSW notification requirements and timing before starting removal work. For urgent work or a late notification, follow SafeWork NSW's current waiver process. This guide is only for the Class B non-friable ARCP template.
Licence holder, removal site, client, notification, and project dates.
Enter the plan date, asbestos licence holder and licence number. Add the removal site, client, SafeWork NSW notification number, start date and expected completion date.
Use the licence search as a check, then compare the result with the current licence record. Make sure the dates match the notification and planned site work.
Consultation records for people and parties notified before work starts.
Add one row for each party informed about the removal. Record the person's name, date, entity, address, how they were informed and any useful notes.
Include the client, workers, other PCBUs, owner or occupant, neighbours and asbestos assessor where they apply. Record what actually happened rather than ticking every suggested party.
First aiders, first aid kit location, and nearest emergency facilities.
Add the first aiders, first aid kit location and nearby emergency facilities. Include a current contact number for each facility.
Check these details against the site emergency plan before work starts.
Use the site plan to show the asbestos removal area, exits, waste storage, decontamination zones, site security, emergency equipment, signage, and services where they apply to the job.

The removal method is split into three stages that match the SafeWork NSW PDF: Preparation, Commencement, and Completion.

Preparation starts empty - add up to two tasks. Commencement and Completion let you add more rows up to the PDF limit for each stage.
On-site storage, disposal site, and EPA WasteLocate consignment details.
Record whether waste will stay on site for more than one working day. If it will, describe the storage arrangements and show the location on the site plan.
Confirm the sealed PPE and RPE arrangements, authorised disposal site and WasteLocate consignment number where they apply. Do not invent a consignment number before one has been issued.
Supervisor and worker declaration signatures.
The declaration says the plan is accurate and was developed in consultation with workers on site. Add each supervisor or worker who signs, with their signature and date.
Do not collect signatures until the signatories have reviewed the plan and the controls that apply to their work.
Who receives or can access a copy of this ARCP.
Record who received the plan and who can readily access it at the workplace. The app covers the person who commissioned the work, other PCBUs, health and safety representatives and residential occupants.
Only select the groups that apply, then make the plan available before work starts.
Tool inspection and HEPA vacuum cleaner maintenance records.
Confirm tools and equipment were inspected before and after the removal work. Record the make, model and last test date for up to two HEPA-filtered vacuum cleaners.
Use the equipment records. Do not estimate a test date.
Asbestos or ACM identified for removal, including inaccessible areas.
Add up to 14 ACM items. For each item, record its description, identification date, exact location, quantity in square metres, condition and condition notes.
Use the asbestos register, survey and site inspection. If the material or condition is uncertain, stop and get the required competent advice rather than guessing.
Personal protective equipment required in the asbestos removal area.
Select the PPE required for this removal area, including coveralls, footwear, overshoes, gloves and eye protection where they apply.
The checklist records the planned PPE. It does not select PPE for you. Base the selection on the job risk assessment, SWMS and current SafeWork NSW guidance.
Respiratory protective equipment requirements for workers.
Record the RPE selected for the job, then confirm the worker fit testing and pre-use fit check arrangements shown in the form.
Check the equipment, filter and face-fit details against the site-specific risk assessment. A tick in this plan is not evidence that fit testing occurred.
Nominated supervisors approved by SafeWork NSW.
Add up to six nominated supervisors with their names and contact numbers. Use the people approved for and available to this job.
Workers involved in the asbestos removal work.
Add the workers involved in the removal work, with a current contact number for each person.
