The form is a four-column site list. It records the product, how it is used, whether it is labelled and whether its safety data sheet (SDS) is available. Open WHS Form 09 when you have the products and current SDS files in front of you.
SafeWork NSW says the workplace register must list applicable hazardous chemicals and include the current SDS for each one. Certain consumer products and chemicals in transit are excluded, so check the current requirements when deciding what belongs in the register.
Add one row for each applicable chemical:
The Tradie Forms PDF has space for 19 rows. If more than 19 applicable chemicals are present, do not leave the rest out. Use another register page or the workplace’s wider chemical-register system and keep the records together.
A No is not a reason to hide the product from the register. It identifies a gap to resolve.
Form 09 records the check. It does not attach the SDS or record the training itself.
Keep the PDF in the WHS site file and keep the current SDS files accessible. Update the register when a new hazardous chemical arrives, a product is removed, or an SDS changes. Make sure affected workers can view the register.
