WHS Form 09 is the hazardous chemicals register from the Housing Industry Site Safety Pack. It lists each product used, stored or handled on the residential site, how it is applied, whether the container is labelled, and whether the current safety data sheet (SDS) is available. The supervisor or PCBU who keeps the site pack completes it when hazardous chemicals are on site; the finished PDF is the SafeWork NSW layout for the file, not a lodged return.

Form 07 for chemical training
The official form tells you to record training, information or instruction in the use of hazardous chemicals on WHS Form 07. Have that register open or planned for the same pack.
Nothing on this form blocks download: every field is optional in the app. A blank register still downloads, so the discipline is whether the PDF matches what is actually on site.
Lists each applicable hazardous chemical for this site pack entry. The official PDF is one page with four columns: Product name, Application, Product labelled? (Yes/No), and SDS available? (Yes/No). The header also reminds you to record training in Form 07.
SafeWork NSW says the workplace register must list applicable hazardous chemicals and include the current SDS for each one. Certain consumer products and chemicals in transit are excluded, so check the current requirements when deciding what belongs here.
One app row is one chemical. Add a row with Add Chemical Row. You can add up to 19 rows, matching the printed table on the official PDF. Leave unused rows out rather than adding blanks. Rows do not reorder; add them in a stable order you can maintain when products change. On the finished PDF each row fills one printed line; long product names and applications can clip, so keep wording tight enough to read in the preview.
If more than 19 applicable chemicals are on site, do not drop the rest. Download this page for the first 19, start another Form 09 for the overflow, and keep both PDFs with the SDS set in the same site file.
The notice at the top of the section is a reminder only: cross-reference WHS Form 07 for worker training on hazardous chemicals used on this site. Form 09 never carries the training details.

Add one row for each applicable hazardous chemical on this workplace. Defaults to no rows, so an empty table downloads as a blank register. Fill every column for each product you add; a name without application or Yes/No answers looks unfinished when a principal checks the pack.
Product Name is the product as printed on the label and the current SDS, not a shorthand or job-site nickname. Copy spelling and grade where the SDS uses them, for example Selleys Liquid Nails High Strength rather than glue. If the container and SDS disagree, resolve it with the supplier before the product is used; do not invent a middle name. Placeholder: Acetone.

Application is how the product is used on this site, not the manufacturer's full use list. Choose from the combobox when it fits (Surface preparation, Cleaning, Adhesive / bonding, Painting / coating, Lubrication, Welding / cutting, Fuel, Concrete additive), or type a custom value when none of those match, such as Form oil for formwork. Keep it specific enough that another trade reading the pack knows why the chemical is there.
Product Labelled? (Yes/No) is a segmented Yes or No. Choose Yes only when the container actually in use is correctly labelled, including decanted workplace containers. Choose No when the label is missing, unreadable, wrong product, or the temporary container has no workplace label. A No is not a reason to hide the product from the register; it is the gap to fix before the product is used. Leaving the control blank is worse than an honest No, because the PDF then shows no check at all.
SDS Available? (Yes/No) is the same Yes/No control for the current safety data sheet. Choose Yes only when the current SDS is readily accessible to workers who use or may be exposed to the chemical, including when access is electronic and people on site know how to open it. Choose No when the SDS is missing, superseded, or locked in an office nobody on site can reach. Obtain the current SDS from the supplier or manufacturer before treating the gap as closed. Form 09 does not attach the SDS file; keep SDS with or linked from the register in the site file.
A No on labelling or SDS does not remove the duty to list the chemical. Fix the label or get the current SDS, then update the row and re-download so the pack shows the corrected check.
Using a site nickname instead of the product name on the label.
Glue, thinners or the red tin cannot be matched to an SDS. Copy the name from the label and the current SDS so anyone reading the pack can find the same product.
Marking SDS available when only an old sheet is in the folder.
The register expects the current SDS. An outdated sheet is a No until the supplier version is on hand and workers can reach it.
Marking labelled Yes when only the original carton is labelled.
The container in use needs the correct label, including anything decanted into a workplace container. Check the tin or bottle on the tools, not the box in the ute.
Leaving a product off the list because labelling or SDS is No.
Form 09 is where the gap is visible. List the chemical, answer No honestly, then fix the label or SDS and update the row.
Writing a catalogue use instead of the site application.
Multi-purpose adhesive for construction does not tell the next reader what is happening on this block. Prefer Timber bonding for stairs or the combobox value that matches the job.
Stopping at 19 rows and ignoring the rest of the store.
The PDF has 19 lines. Overflow needs another Form 09 page or the workplace's wider chemical-register system, kept with this pack. Dropping products makes the register look complete when it is not.
Treating Form 09 as the training record.
The official form points training, information and instruction to Form 07. A chemicals list alone does not show who was instructed on the products.
Keep the finished PDF in the WHS site file on site and make sure people affected by the chemicals can view the register. Keep the current SDS for each listed chemical with or readily accessible from the register. Tradie Forms does not lodge Form 09 with SafeWork NSW or any other authority.
Update the register when stock changes. Add a row when a new hazardous chemical arrives, remove or strike products no longer on site when you re-download a current list, and refresh SDS Yes/No when a sheet is replaced.
Name the file with the business and date so it sits with Forms 01 to 10 for the same site, for example WHS-09 - Mitchell Building - 2026-06-15.pdf.
File SDS with the register, not only in a supplier portal nobody on site can open. If access is electronic, workers still need a reliable way to open the current sheet on site.
Cross-check Form 07 so workers who use the listed chemicals have training, information or instruction recorded where that is required for the job.
Feed Form 04 and Form 05 when chemicals change the risk picture. Products that affect atmosphere, fire risk, or handling should appear in the site risk assessment and any SWMS that covers that work.
