Use this guide to complete WHS Form 09: Hazardous Chemicals Register from the SafeWork NSW Housing Industry Site Safety Pack.

The form is a four-column site list. It records the product, how it is used, whether it is labelled and whether its safety data sheet (SDS) is available. Open WHS Form 09 when you have the products and current SDS files in front of you.

Before you start

• Walk the storage and work areas and identify hazardous chemicals used, stored or handled at the workplace
• Use the product label and current SDS to confirm the exact product name
• Check that decanted containers and workplace containers are labelled correctly
• Make the current SDS readily accessible to workers who use or may be exposed to the chemical
• Record relevant training, information or instruction in WHS Form 07

SafeWork NSW says the workplace register must list applicable hazardous chemicals and include the current SDS for each one. Certain consumer products and chemicals in transit are excluded, so check the current requirements when deciding what belongs in the register.

Hazardous Chemicals

Add one row for each applicable chemical:

• Product name - copy the name from the label and SDS
• Application - say what it is used for on this site, such as surface preparation, bonding or cleaning
• Product labelled? - choose Yes only when the container in use is correctly labelled
• SDS available? - choose Yes only when the current SDS is accessible to the workers who need it

The Tradie Forms PDF has space for 19 rows. If more than 19 applicable chemicals are present, do not leave the rest out. Use another register page or the workplace’s wider chemical-register system and keep the records together.

What the Yes and No answers mean

A No is not a reason to hide the product from the register. It identifies a gap to resolve.

• If the product is not labelled correctly, check the original container and current labelling requirements before it is used
• If the SDS is missing or out of date, obtain the current SDS from the supplier or manufacturer
• Make sure workers know where the SDS can be found, including when access is electronic

Form 09 records the check. It does not attach the SDS or record the training itself.

Check before download

1. Product names match the label and SDS
2. Applications describe how each product is used on this site
3. Label and SDS answers reflect what is actually available
4. Every applicable chemical is included, using extra pages if needed
5. Current SDS files are kept with or readily accessible from the register
6. Worker training, information or instruction is recorded in Form 07 where relevant

After download

Keep the PDF in the WHS site file and keep the current SDS files accessible. Update the register when a new hazardous chemical arrives, a product is removed, or an SDS changes. Make sure affected workers can view the register.
