How to complete WHS Form 01 organisation details in Tradie Forms.

WHS Form 01 records the business, insurance, contact and subcontracting details that open the Housing Industry Site Safety Pack. The PCBU or site manager fills it when setting up the site safety file for a residential construction job. The finished PDF is the SafeWork NSW layout; keep it on site with the rest of the pack and attach current insurance certificates of currency where the principal asks for them.
This is the current SafeWork NSW WHS Form 01 from the Housing Industry Site Safety Pack. Tradie Forms follows the official section order and maps your answers onto the official PDF layout.
Almost every Form 01 problem comes from typing from memory instead of from current business and policy records. Have these in front of you.
Your business identity
Trading name, ACN or ABN, and the contract licence number you work under.
Current insurance certificates of currency
Workers compensation, public liability, professional indemnity, and sickness and accident where you hold them. The form is not proof that a policy is current.
Director or manager and site WHS contact
Who signs, and who is responsible for managing WHS on this site, with a number that reaches them.
A decision on subcontracting
Whether you will subcontract any of the works, and if so the intended subcontractor business and contact.
Optional company logo
A clear JPG or PNG if you want it stamped into the logo box on the PDF.
Business or trading name and the sign-off date gate the download.
Captures the business that owns this site pack entry: who you are, how many people you run, and what work this pack covers.
The name on your ABN record and insurance certificates. Use the same spelling across every HIISP form for this job so the principal's file reads as one pack, not three different companies. If you trade under a different name from the legal entity, use the trading name the site knows you by and keep the ABN consistent.
Upload a JPG or PNG. The logo stamps into the logo box on the downloaded PDF. Skip it if you do not have a clean file on the phone; blank is better than a stretched screenshot.
From your ABN record or the header of a current certificate of currency. Use ABN lookup to fill the business name when the number is known. Check the result against your licence and insurance, because a group structure can return a related entity rather than the one on site.
Headcount for the business, not the crew on this one site. Use the figure you would give a principal or insurer if they asked today.
The work this business does under the pack, written so a principal reading the site file understands the trade without opening the contract. Name the trade and the usual residential work types.

Four rows on the official form: Workers compensation, Public liability, Professional indemnity, and Sickness and accident. Each row uses the same five columns. Leave a row blank when that cover does not apply. Copy every value from a current certificate of currency or policy schedule, not from last year's site pack.
The form notes that you should attach copies of certificates of currency. The PDF fields alone are not proof that a policy is still in force.
The insurer or underwriting organisation named on the certificate, such as GIO or QBE. Same column on every insurance row.
The policy or certificate number on the currency document. Do not invent a job reference here; a number in this column reads as the insurer's own.
The per-claim limit from the certificate, in dollars. The field shows a $ prefix; enter the figure as on the schedule, for example 10,000,000.
The aggregate or annual limit where the certificate states one. If the schedule only shows a per-claim limit, leave this blank rather than repeating the same figure by guesswork.
The expiry on the current certificate. The form only accepts today or a future date, so an already-expired policy cannot be entered as current. When a policy is due to renew mid-job, update this row and re-download once the new certificate arrives.
Licence, director, business address, phones, and the person who manages WHS on this site. These are the lines a principal dials when something goes wrong.
Your NSW contractor licence number as printed on the licence, including the class letter when it forms part of the number, such as 123456C. Take it from the licence card or the NSW Fair Trading record, not from a quote footer that may lag a renewal.
The person who will sign at the bottom of the form. Match the name on the sign-off signature so the pack does not show two different people in charge.
The business address, not the job site. Address search is limited to New South Wales and fills the street line, suburb, state and postcode. Check the result against your letterhead or ABN record if the business has moved recently.
Filled by address search. Confirm NSW when you type the address by hand.

The office landline as you publish it to principals, for example 02 9123 4567.
A mobile that is answered during site hours. Prefer the number that reaches the director or office, not a site-only burner that changes every job.
Only when you still publish a fax number. Leave blank rather than inventing one to fill the box.
The inbox that receives site pack and insurance requests. Use a monitored business address, not a personal account that leaves with a staff member.
The named person who owns WHS on this site day to day. Can be the director, a site manager, or another appointed person. This is who the principal and SafeWork will expect to speak to on site.
How to reach that WHS person quickly, usually a mobile number. Put the number that works on site, not only the office switchboard.
Records whether you intend to subcontract all or part of the works on this site, and who you intend to use. The official form also states that you must ensure those subcontractors provide an SWMS for specialised high risk construction work, and that you will participate in reviewing and amending that SWMS with the PCBU and the workers it covers.
Choose do or do not. Answer for this site, not for the business in general. Leaving it blank makes the pack look unfinished when a principal checks the first page.
Required in practice when you selected do. Name the intended subcontractor business as it will appear on their SWMS and insurance, for example Spark Right Electrical Pty Ltd.
A named person and number for that subcontractor, such as Sam Lee, 0400 111 222. Enough that the site can reach them without opening another file.
If more than one subcontractor is intended, put the primary or first-engaged business here and keep the full subcontractor list with their SWMS in the site file. Form 01 is a single pair of business and contact lines on the official PDF.
Director or manager signature and date. This closes Form 01 on the official layout.
The director or manager named above. Sign when the organisation, insurance and contact details are correct for this site pack, not as a blank template for later jobs. You can use a saved signature; check the preview so the stamp sits cleanly in the signature box on page one.
The date this Form 01 is completed for the pack. Today and Yesterday shortcuts are there because this is usually filled when the site file is opened. The field does not accept a future date.
Treating the insurance rows as proof of cover.
The form tells you to attach certificates of currency. Policy numbers alone do not prove a policy is still current when a principal or SafeWork checks the file.
Copying last year's site pack without checking renewals.
Trading names, licence numbers and insurance expiry dates change. A pack that still shows a lapsed policy or an old licence is worse than an incomplete page, because it looks current.
Selecting do for subcontracting and leaving Business blank.
The official form expects the intended subcontractor when you will use one. Empty business and contact lines under do read as unfinished.
Using a different business name on every HIISP form.
Form 01 sets the identity for the pack. Forms 04, 05 and 06 that show another trading name make the file look like three contractors, not one.
Putting the job site address in Business address.
Business address is your office or registered place of business. The site address belongs on the site-specific forms later in the pack.
Naming one person as director and signing as someone else.
Name of director or manager and the signature should match. Two different names on the same page undermine who is accountable for the pack.
Leaving Person responsible for managing WHS on site empty.
That is the contact a principal or inspector expects on the ground. If the director is not on site daily, name the person who is.
Keep the finished PDF in the site safety file on site and retain a copy with your business records. Tradie Forms does not lodge Form 01 with SafeWork NSW or any other authority.
