WHS Form 02 records who holds overall WHS responsibility for the business, and the business owner and supervisor sign-offs that go with those roles. It is the second page of the Housing Industry Site Safety Pack, filled by the PCBU or site manager when the site safety file is set up. The finished PDF is the SafeWork NSW layout, with the duty lists already printed under each role. If you still need the business identity page, use NSW WHS Organisation (Form 01).

This is the current SafeWork NSW WHS Form 02 from the Housing Industry Site Safety Pack. Tradie Forms follows the official section order and maps your answers onto the official PDF layout.

Signing this page records who is allocated which role on the pack. It does not transfer or remove duties under the current WHS requirements. The people named still need the authority and competence to carry out the role.

Before you start

Duty bullet lists for business owner, supervisor and worker are already on the PDF. You fill the overview line and the two sign-off rows only. Have these in front of you.

  • Who has overall WHS responsibility

    The role title and the named person or business that owns safe work for this site pack.

  • Business owner or director ready to sign

    Name of the owner, director or manager who will sign the first row, and a way to capture their signature.

  • Supervisor or leading hand ready to sign

    Name of the manager, supervisor or leading hand who runs operational safety on site, and their signature.

  • The date the pack is being completed

    Both sign-off rows need a date. Use the day each person signs, not a future planning date.

Six fields gate the download: role or title, responsible person or business, both names, and both dates. Signatures are strongly expected on a finished pack page even though download does not block without them.

Overview

Builds the opening sentence on the PDF: who has overall responsibility for the safe conduct of all work carried out on behalf of the business. Use the actual arrangement for this job, not a generic company line.

Role or TitleComboboxRequired

The position that holds overall WHS responsibility for work done under this business. Pick from the list (Director, Site Supervisor, Project Manager, WHS Manager, Foreman, Leading Hand, Worker) or type a custom title if none fit. This fills the first blank in the PDF sentence, so write the role as it is used on site, not a job code.

Responsible Person or BusinessTextRequired

The named person or the business that holds that overall responsibility. Match the trading name on Form 01 when the business itself owns the duty, or the real person when an individual has accepted it. A vague entry here leaves the principal's file without a clear account holder.

Business Owner

Owner, director or manager sign-off. The duty list under this heading (implementing procedures, planning work safely, training, consultation, hazard reports, and so on) is printed on the downloaded PDF. You only record who accepts that role and when.

Full name of the owner, director or manager signing this row. Prefer the same person named as director or manager on Form 01 so the pack does not show two different people in charge.

SignatureSignatureOptional

The same person named above. Sign when the overview and name are correct for this site pack. You can use a saved signature; check the preview so the stamp sits cleanly in the signature box on the page. Download does not block without a signature, but a blank signature box on a handed-over pack reads as incomplete.

DateDateRequired

The day this person signs. Today and Yesterday shortcuts are there because this is usually filled when the site file is opened. The field does not accept a future date.

Supervisor

Manager, supervisor or leading hand sign-off. The printed duty list covers operational safety: SWMS facilitation, site inductions, toolbox talks, inspections, and day-to-day compliance. Record who holds that operational role on this job.

Full name of the manager, supervisor or leading hand who will carry operational safety on site. If the owner also runs the site day to day, the same person can appear on both rows; still complete both sign-offs when that is the real arrangement.

SignatureSignatureOptional

The same person named in this section. Sign when they have accepted the operational role for this pack. Use a saved signature if you have one, and check the preview for placement in the lower signature box.

DateDateRequired

The day this person signs. Same rules as the business owner date: no future dates, Today and Yesterday shortcuts available. When both people sign on different days, use each person's actual sign date rather than forcing a single pack date.

Worker

Individual worker duties are printed on the downloaded PDF. There are no fields to fill in this section.

The PDF states that every worker is responsible for conducting allocated tasks safely, considering others, co-operating on WHS, leaving the work site safe, complying with the SWMS (and being encouraged to help develop SWMS for high risk construction work), and complying with site safety rules from the site induction. Brief the crew against those points; do not try to retype them into this form.

Common mistakes

  • Putting a role title that nobody on site holds.

    The overview sentence names who has overall responsibility. If the pack says WHS Manager and the job has none, a principal or inspector will ask for someone who is not there. Match the real arrangement.

  • Writing a vague responsible person line.

    the company, TBA or a first name alone does not identify who owns the duty on the finished PDF. Use the trading name from Form 01 or the full name of the person who has accepted the role.

  • Naming one person and signing as someone else.

    Name and signature on each row should match. Two different identities on the same sign-off undermine who accepted the duty list printed above it.

  • Leaving both signatures blank because download allowed it.

    Names and dates alone leave empty signature boxes on the SafeWork layout. Capture both signatures before you file the page in the site pack.

  • Using a future date so the pack looks ready early.

    Both date fields reject future dates. Sign when the allocation is real, not when you hope the job will start.

  • Treating the signed page as a transfer of legal duty.

    Completing Form 02 records the allocation on the pack. It does not remove WHS duties from the PCBU or anyone else under the legislation. Keep competence and authority aligned with the names you write.

  • Skipping the briefing for the people named.

    A signed PDF the supervisor has never seen is paperwork, not a working arrangement. Walk each signatory through the duty list printed under their heading before you file the page.

Keep the finished PDF in the site safety file on site and retain a copy with your business records. Tradie Forms does not lodge Form 02 with SafeWork NSW or any other authority.

  • Name the file with the business and date so it sits with Forms 01 and 03 for the same site, for example WHS-02 - Mitchell Building - 2026-06-15.pdf.
  • Brief the people named on the duty lists printed under their headings before you leave the pack on site.
  • Reuse the Business Owner and Supervisor saved details on the next pack for the same people, then re-check that the overview still matches this job.
  • Move on to the rest of the pack the job needs. Form 02 alone does not replace the WHS policy, risk assessment, SWMS, toolbox talks or registers.
