How to complete the Housing Industry Site Safety Pack worker training register in Tradie Forms.

WHS Form 07 records one worker's training, instruction and licence evidence for the Housing Industry Site Safety Pack. The PCBU or site manager fills a separate register for each person on site so the pack can show who holds which tickets before high risk work starts. The finished PDF is the SafeWork NSW single-page layout with space for up to 13 training rows.
This is the current SafeWork NSW WHS Form 07 from the Housing Industry Site Safety Pack. Tradie Forms follows the official section order and maps your answers onto the official PDF layout.
Almost every Form 07 problem comes from typing from memory instead of from the card, certificate or induction record in hand. Have these in front of you.
One worker per form
Name and employment start date for this person only. A second worker needs a second register.
Current cards and licences
White card, high risk work licences, trade or contractor licences, first aid certificates, and any other tickets they will rely on for this site.
Induction and course records
Provider names, course titles, and issue dates for WHS general induction, site-specific induction, hazardous chemicals training, and work activity training.
Expiry and renewal dates
The date printed on each card or certificate that renews. Leave the field blank only when the evidence has no expiry, not when you cannot find it.
Which tickets this site needs
Match the rows to the high risk tasks and plant on Form 05 and the chemicals on Form 09 for this job, not a generic company list.
Worker Name gates the download. Training rows are optional in the form, but an empty register is usually useless to a principal checking the pack.
Identifies the person this register covers. Every training row below belongs to this name alone.
The worker's name as it appears on their white card, high risk work licence, or employment record. Use the same spelling across Form 05 worker sign-off and Form 06 attendance so the pack reads as one person, not three variants. If you only have a nickname on site, take the legal or licence name from the card before you download.
The day this worker started with your business, from the employment record or induction file. The field does not accept a future date. Fill it when you have it; a blank start date is common on casual starts but weakens the trail when a principal asks how long the person has been with you.

Records each piece of training, instruction or licence evidence for this worker. One row is one ticket, course or induction event. The official PDF also lists example types in a notice: WHS general induction for construction work, high risk work licences, load shifting, use of hazardous chemicals, trade or contractor licence, work activity training, tools and equipment, site-specific induction, and induction booklet.
You can add up to 13 rows. That matches the fixed table on the official one-page PDF. Extra rows beyond 13 do not fit the layout, so start a second Form 07 for the same worker if you need more lines, and keep both PDFs together. Leave unused rows out rather than adding blanks. Rows do not reorder; add the important site tickets first so they stay visible on the page.
Each row has five columns. Copy every value from the evidence you checked, not from last year's pack or a photo of someone else's card.
Add a row for each relevant ticket or induction. Type of Training, Instruction and/or Information is a combobox with common options (WHS general induction, site-specific induction, high risk work licence - forklift, working at heights, confined space entry, asbestos awareness, first aid, electrical isolation / LOTO) and you can type a custom value when none of the options fit. Name the licence class or course clearly enough that a principal reading the pack knows what was checked, for example High risk work licence - forklift, not ticket.
Licence or Card Number is the number printed on the card or certificate when there is one, such as a white card number or high risk work licence number. Leave it blank when the evidence has no number (for example a site toolbox induction with no card). Do not invent a job reference here; a number in this column reads as the authority's or provider's own.
Person Providing Training is the RTO, registered training organisation, SafeWork NSW, site supervisor, or company that delivered the training or issued the card. Use the name on the certificate, such as Site Skills Training or SafeWork NSW.
Date Issued is the issue date on the evidence. The field does not accept a future date. Use Today or Yesterday only when that matches the card.
Expiry or Renewal Date is the expiry printed on the licence or card when it renews. The field only accepts today or a future date, so an already-lapsed ticket cannot be entered as current. Leave it blank when the evidence genuinely has no expiry. When a licence is due to renew mid-job, update the row and re-download once the new card arrives.

Prefer the tickets this worker will use on this site: white card and site induction for everyone, then high risk licences for plant and heights work named on Form 05, first aid if they are the nominated first aider, and hazardous chemicals training when Form 09 products apply. A long list of unrelated historical courses that never touch this job buries the lines a principal actually checks.
Putting two workers on one register.
The official form is one name and one employment start date. A second person needs a second PDF. Combined names make it impossible to prove who holds which ticket.
Typing licence numbers from memory.
Transposed digits pass a quick glance and fail when the principal checks the card. Copy the number from the physical licence or the provider certificate in front of you.
Leaving expiry blank on a renewing licence.
High risk work licences and many first aid certificates carry an expiry. A blank expiry column next to a class that always renews reads as unfinished or out of date.
Entering a past expiry as still current.
The form blocks past expiry dates for a reason. If the card has lapsed, do not force a date into the field. Arrange renewal or keep that worker off the plant until the new card is in hand.
Writing white card or ticket with no detail.
The type column and licence number exist so the pack shows what was checked. Name the induction or licence class and include the card number when the evidence has one.
Copying another worker's rows onto this form.
Crew share plant and training providers; they do not share licence numbers. Prefill the Worker saved detail for the same person only, then re-enter tickets from that person's own cards.
Leaving the register empty for workers named on high risk work.
Form 05 and Form 07 sit together in the pack. A blank training page for someone listed against high risk construction work is the gap a principal or inspector notices first.
