Work Will Commence On
Date work will start on site. Lodge at least 24 hours before eligible jobs commence.
Business Details
Business name, ABN, postal address, and contact details for the notifier.
Gasfitter Details
Licensed gasfitter name, licence number, and contact details. Use the snippet toolbar to save and reuse your details.
Gas Works Location
Premises or caravan address, owner name, and site contact details.
Nature of Work
Tick each type of gas work that applies: new installation, addition, alteration, repair, containers, type B, appliances, or removal.
Type of Installation
Industrial, commercial, domestic, dispensing, or bulk storage.
Gas Withdrawal
Liquid, low pressure vapour, or high pressure vapour, plus work pressure details.
Type of Gas
LPG or natural gas.
Gas Source
Cylinder or bulk tank supply.
Purpose of System
Describe what the gas system is used for and what it is connected to.
Piping Sizing
Gas usage, pressure drop, main run, high-pressure stage values, and sizing table reference. Include calculations on the site plan when using formulae.
Gas Storage Tank
Capacity, manufacture, serial number, plant item number, and expiry date for each storage tank row.
Gas Container/s
Capacity, manufacturer, ID number, manufacture date, and test date for each container row.
Compliance Checklist
Tick items where work conforms to the Dangerous Goods Regulations, including AS1596, AS5601, AS3814, and AS4645.
Site Plan and Installation
Provide installation plan particulars as written details, a diagram, or a photo upload, including ignition sources, protected works, and distances from the gas container.
Notifier Declaration
Declare the information is true and correct, tick electronic submission if applicable, sign, and date.
After export
Lodge completed notifications in person, by email, or by post at NT WorkSafe. Keep a copy for your records.
