How to complete Queensland Form 14 compliance declarations in Tradie Forms.

Form 14 is the compliance declaration that the completed plumbing or drainage work matches the action notice and the Plumbing and Drainage Regulation 2019. The responsible person fills it and gives it to the local government so the notice can be closed out against the permit file. It is for that declaration pathway under section 79(2)(c), not a remote area completion notice or a final inspection certificate.

Tradie Forms prepares the declaration. It does not decide whether the work complies or close the action notice.

Before you start

Almost every Form 14 problem comes from filling it from memory or a builder email instead of from the permit and the action notice. Have these in front of you.

  • The current permit and any amendments

    The source for the land description, permit number and permit issue date.

  • The action notice

    The reference, issue date, stated work and the period for giving the declaration to the local government.

  • What was actually finished

    A short description of the plumbing or drainage completed, in the same language as the permit and the notice.

  • The real completion date

    The day the declared work was finished on site, not the day you are lodging the declaration.

  • Responsible person details

    Full name, occupational licence, contractor licence if held, phone, email and postal address for the licensed person who performed or supervised the work.

  • Contractor details when different

    Company or individual name, contractor licence and contact details when the responsible person is not the contractor.

Only the declaration date blocks download on an untouched form. Signature, land, permit, action notice, work and people details still need to be complete for a declaration the local government can accept.

Property

Identifies all land the declaration covers. The official form is used for section 79(2)(c) of the Plumbing and Drainage Regulation 2019, and the land description must identify all land the subject of the declaration. Lot and plan details (for example SP or RP) are on title documents or a rates notice.

Street addressAddressOptional

The address on the permit and the action notice. Address search is limited to Queensland and fills the street line, suburb, state and postcode. Check the result against the permit, because search returns the current postal address and a permit can predate a street renaming or a subdivision. Do not paste a delivery or job-system address without that check.

Lot and PlanTextOptional

The legal land description from the permit, title documents or rates notice. On new estates the street address may not exist in any council system yet, so lot and plan is often the only reliable identifier. Fill it whenever the permit carries it.

Fills from the street address search. Edit it when you identify the land by lot and plan only and the street postcode is blank or wrong.

Shop / Tenancy NumberTextOptional

Fill this on multi-tenancy sites so the declaration ties to the part of the premises the notice covered.

Storey / LevelTextOptional

Fill this on anything above one storey, for the same reason.

Local Government AreaTextOptional

The council area that receives the declaration. Address search may fill this. Confirm it near a boundary, because the LGA is who the action notice belongs to.

Check the finished property block against the permit and the action notice before you move on.

Permit

Links the declaration to the plumbing permit the work was done under. The form notes that a person must not carry out permit work unless they have a permit for the work (section 66(1) of the Plumbing and Drainage Act 2018).

Permit NumberTextOptional

From the current permit document. Do not take it from an invoice, a quote, an old Form 1 or a different stage of the same job, because those often carry a superseded number after an amendment. If you are on site without the permit, the permit holder or the council's plumbing team can confirm it.

Date Permit Issued (if Known)DateOptional

The issue date on that same permit. Fill it when you have it: it is how the local government confirms you declared against the current permit rather than a replaced one. The field cannot be set to a future date.

If the permit was amended during the job, use the number the local government still holds for the work being declared.

Action Notice

Records the action notice that requires this declaration. Form 14's declaration wording refers to conformity with the relevant action notice, so this block is the scope of what you are closing out, not a spare reference field.

Reference NumberTextOptional

The action notice reference from the notice itself, not from a job note, email subject line or chat photo that paraphrases it. Do not put this number in the permit field because the notice was the last document you opened.

Date Issued (if Known)DateOptional

The issue date on that same notice. Check it against the notice face, not the day you first saw a copy. The field cannot be set to a future date.

Read the notice for the stated work, required action and the period for giving the declaration to the local government before you write the work description. Keep the issued notice with the finished PDF; a reference alone does not tell the story later.

Work

Describes the plumbing work that has been performed. The official form says the description must be sufficient to identify the work, for example installation of plumbing and drainage associated with a new domestic dwelling.

Description of the Work PerformedTextOptional

A brief description of the completed plumbing or drainage in the same terms as the permit and the action notice, not the quote title or stage code alone. Name the systems or installation and, where it helps, the part of the premises. On the PDF the text is split across three lines of about 90 characters each, so keep it short enough to read on the page without crushing the part that identifies the work.

Describe only the work this declaration covers. If scope changed on site, confirm with the responsible person what is complete for the purpose of the notice before you write the line.

Completion

Records when the declared work finished. That date is a different event from the day you sign the declaration.

Date the Work Was CompletedDateOptional

The date the declared work was actually finished on site, not a booking date, promised handover or invoice day. Today and Yesterday shortcuts are there because this is usually filled on site or the next morning. The field cannot be set to a future date.

Responsible Person

The licensed person who performs the work, or supervises its performance. Their name on this block is the person standing behind the declaration.

Name (in Full)TextOptional

Legal full name of the responsible person as on their occupational licence, not a trading name or company name. Licence check can fill the name when you look up a QBCC licence number.

Occupational Licence NumberTextOptional

The plumber or drainer occupational licence number. Licence check against the QBCC register can fill the name; confirm the result matches the person who did or supervised this work.

Contractor Licence Number (if Applicable)TextOptional

Fill when the responsible person also holds a contractor licence for the work. Leave blank when only the occupational licence applies and the contractor sits in the next section.

Phone NumberTelOptional

A number the local government can use if the declaration needs a quick check.

Email AddressEmailOptional

The contact email for the responsible person or their office on this job.

Postal addressAddressOptional

Postal address for the responsible person. Address search is limited to Queensland. Check the filled street, suburb, state and postcode against the licence or business records you keep for lodgement.

Save this section as Responsible Person to prefill it on your next Form 14 for the same licensed person.

Check licence numbers against current QBCC and occupational licence records before you sign.

Contractor

Complete this section when the responsible person is not the contractor for the work. Leave it blank when the responsible person is also the contractor.

Full Name of Company (or Individual if Not a Company)TextOptional

Company name as on the contractor licence, or the individual's full name when the contractor is not a company. Licence check can fill the name from a QBCC contractor licence number.

The contractor licence for the work. Use Licence check against the QBCC register to confirm the number and fill the company or individual name.

A contact number for the contractor on this job.

The contractor email used for this permit file.

Save this section as Contractor to prefill it on your next Form 14 where the same contractor applies.

Do not copy a business contact into both people blocks when the roles differ on this job.

Declaration

Signs the statement that the work has been completed in conformity with the relevant action notice and the Plumbing and Drainage Regulation 2019, that the information on the form is a true and accurate record, and that the signatory is the responsible person.

Sign as the responsible person named above, after the property, permit, action notice, work, completion date and people blocks are correct. You can use a saved signature; check the preview so the stamp sits cleanly in the signature box on the single-page PDF. Do not sign for someone else, and do not sign while the declared work is still incomplete.

The date the declaration is made. Today and Yesterday shortcuts are there for on-site completion. The field cannot be set to a future date. This is a different event from the completion date when you sign after finishing site work, and it is the field that blocks download when left empty.

Read the declaration wording on the form before you sign. Confirm the land, permit, action notice reference, work description and completion date match the job you are closing out.

Common mistakes

  • Taking the permit number from an invoice or old Form 1.

    Invoices and early job paperwork often carry a number that was later amended. Take the number and issue date from the current permit document.

  • Putting the action notice reference in the permit field.

    Permit and action notice are separate sections. A notice number in the permit field points the declaration at the wrong council record.

  • Describing only part of the land.

    The declaration must identify all land it covers. On units, multi-building sites, shop tenancies or staged work, add lot and plan, tenancy or level from the permit rather than the delivery address alone.

  • Writing a vague work description.

    Plumbing works complete does not distinguish this job months later. Name the systems and location in the same terms as the permit and the action notice.

  • Using a booking or invoice day as the completion date.

    The completion date is when the declared work was actually finished on site. The declaration date is when you signed.

  • Copying responsible person and contractor from the last job.

    The roles are separate, and stale licence details are a common rework. Confirm the person, licence numbers and contacts for this work before you sign.

  • Signing before the form is complete.

    A signed declaration with a blank permit, notice or completion field creates rework under pressure. Preview the page, fix mismatches, then sign.

Give the finished PDF to the local government as the action notice requires, within any period the notice states. Keep a copy with the notice, the permit and the supporting job records rather than leaving it in downloads.

  • Name the file with the site and notice or permit. Form 14 - 12 Cable St - AN-2024-00182.pdf is easier to find than a generic download name when council asks for a re-send.
  • File it with the permit, the action notice and any amendments. The declaration alone does not show what was required. The set is what answers a question later.
  • Keep the lodgement record. Note how and when you gave the declaration, and keep any receipt, email or portal reference with the job folder.
  • Keep testing and inspection trail with it. Form 14 does not replace Form 5, Form 9, site notes or photos that belong to the same job.
