Before you start

Almost every weak smoke alarm record comes from writing it up after leaving the property. Have these in hand before you open the form.

  • Access to every alarm

    Every bedroom, hallway, level, and any other location required for this property type, so no unit is missed on the register.

  • Alarm markings you can read

    Make and model, power source, and replace-by or manufacture date on each unit, read while you are under it.

  • Test results and actions

    Pass or fail for each alarm, interconnection results, and what you did on fails (replaced, battery swap, cleaned, agent notified).

  • Property and agent details

    Full site address, agent or client name, agency, and any tenancy or job reference the recipient files against.

  • Service type and rule set

    Why you are on site (annual, tenancy change, install, callout) and the legislation or standard this visit follows.

  • Business and technician details

    Trading name, technician name, and licence or competency number when the work is licensed electrical work.

Technician NameTextRequired

The person who inspected and tested the alarms on this visit, for example Alex Smith. This is the name the declaration can mirror later.

Licence or Competency No.TextOptional

Include it when the visit involved licensed electrical work such as replacing a hardwired alarm, or when the client or site requires it. Leave blank on a battery-only round where no licence is expected rather than inventing a number.

PhoneTelOptional

Contact number for follow-up or a return visit. Use the number the agent already has for you when that is different from a personal mobile.

Property

Where the service was done and who the finished record goes back to. The address is what the PDF hangs on months later when an agent matches it to a tenancy file.

Who receives the finished PDF. On rentals this is usually the property manager rather than the tenant, for example Mia Chen.

Agency or client company name when it differs from the person, for example Coral Coast Property Management.

Tenancy ReferenceTextOptional

Job, work order, or tenancy file reference the recipient already uses, such as CC-4870-008. It prints in the register summary so a long PDF can be matched without opening the agency system.

Frames the visit: why you attended, when, under which rules, and how next-due dates should be suggested on new alarm rows.

Legislation or StandardTextOptional

The rule set this service was performed under, written as the Act, regulation, or standard you actually used. AS 3786 is the product standard for the alarms themselves; tenancy and interconnection duties sit in state law. Example: Queensland Fire Services Act 1990. The form does not choose a state requirement for you.

Default Next Service IntervalSelectOptional

12 months or 6 months. Suggests next-due dates on new alarm rows when you add them. It is a date helper, not a compliance decision. Change any suggested date when the building schedule or applicable rule calls for something different.

Register

Every alarm inspected and tested on this record. One card per alarm. Add a row with Add alarm while you are still under that unit so location, markings, and results match the hardware in front of you.

New rows carry the previous test date only. Location is not carried forward, because every alarm is in a different place. Use Duplicate for similar units: it copies make and model, alarm type, power source, replace-by date, and interconnection result, and clears the location, overall result, dates, action, and notes. Leave unused rows out rather than adding blanks.

On the finished PDF each alarm is a stacked record rather than a wide table. Long registers continue across pages with one consistent footer and page count. The register summary also carries the property reference, service type, and legislation from the sections above.

The register itself is optional on an empty form, but a download with no alarms is rarely useful to an agent. Once you start a row, the form blocks download until that row is complete.

Alarm Location - required on each row. Specific enough that the next technician finds the same unit, for example Hallway outside bedrooms or Bedroom 2, not Upstairs.

Make and Model - optional. Read it off the unit, for example SafeHome SA10-RF. Helps match replacements on the next visit.

Alarm Type - required. Photoelectric, Ionisation, Heat alarm, or a custom value. Photoelectric is what new and replacement alarms are required to be under many current state rules; recording type is how a swap from an old ionisation unit is documented.

Power Source - required. 240V with battery backup, 10-year lithium battery, 9V battery, or a custom value. Hardwired replacement is licensed electrical work.

Replace-by Date - optional. Use the replacement date shown by the manufacturer or required by the applicable rules. Most domestic alarms are replaced about 10 years from manufacture. Leave blank when the marking is unreadable rather than guessing, and note the condition in the failure details or notes when that matters.

Interconnected - required. Yes, No, or Not applicable. Testing that one alarm sets off the others is separate from testing that it sounds at all, so this is its own result.

Result - Pass or Fail. Required once the row is started.

Test Date - required. Defaults to today on new rows when that option is on; change it when you are writing up a visit from the previous day.

Next Due - required when the result is Pass. The default service interval can suggest this date; confirm it against the property schedule and local rules.

Action and failure details - required when the result is Fail. Suggested actions are Alarm replaced, Battery replaced, Cleaned and retested, and Agent or landlord notified. A fail with no action is an open loop. An optional photo on fail is available when the agent needs evidence of the unit you removed or fitted.

Sign off that the alarms listed were inspected and tested as recorded. Complete the register first so you are not standing behind a blank interconnection result or a missing fail action.

Anything the agent or landlord should know that does not fit a single alarm row, such as access you could not get, a follow-up booked to replace a unit, or that an expired kitchen alarm was replaced and the agent advised on completion. Notes are limited on the PDF, so keep them short and specific.

Sign on screen or use a saved signature. The signature attests that the listed alarms were inspected and tested as recorded. Check the preview so the signature sits cleanly in the declaration block.

Common mistakes

  • Writing locations as Upstairs or Alarms OK.

    The next visit cannot find the same unit, and an agent cannot answer a tenant dispute. Name the room or hallway for every row.

  • Guessing a replace-by date when the marking is unreadable.

    An invented date looks like manufacturer evidence. Leave replace-by blank and put the condition in the failure details or notes instead.

  • Marking Pass without a next-due date.

    The form blocks download for that reason. A pass with no next service date leaves the owner without a clear retest point.

  • Recording Fail with no action or failure details.

    A fail that looks open lands back on you. Choose the real action and say what was wrong or what was fitted.

  • Treating interconnection as the same as the sound test.

    An alarm can sound and still fail to trigger the others. Interconnection needs its own result on every row where it applies.

  • Filling the register from memory in the ute.

    Locations swap, replace-by dates blur, and fails lose their action notes. Complete each card under the alarm before you move on.

  • Leaving legislation blank on a tenancy-change visit.

    Agents file this PDF as proof of a legal duty. Name the rule set you worked under so the reason for the visit sits on the page.

Give a copy to the agent, landlord, or owner and keep one with the property or tenancy file. Tradie Forms does not lodge this record with any authority.
