It captures the certifier, property owner, work site, relationship, scope, insurance answers, and owner sign-off on the CBOS layout so you can lodge the finished PDF. Fill and lodge it before the unpaid work starts, not after everyone has left site.

Declares whether you or the property owner hold current liability cover at the amounts stated on the form.
The form wording refers to liability for personal injury and damage to property for at least $5,000,000 for any one occurrence for public liability, and in the annual total for products liability. Answer against that wording and the real policy position.

Select Yes or No. Do not guess from habit or from a different job's policy. If the owner is answering part of the insurance picture, make sure they understand what is being declared. A blank answer blocks download; a wrong answer is worse because the finished PDF looks complete. Confirm against the current certificate of currency before you choose.

The owner signs after reviewing certifier details, owner name, work site, relationship, prescribed work description, and insurance answers. Capture it while they are available. You can use a saved signature only when it is genuinely the owner's mark for this form; check the preview so the stamp sits cleanly in the signature box on the PDF.

The date the owner signs. Today and Yesterday shortcuts are available for on-site capture. The field does not accept a future date. Do not backdate or use a planned work date instead of the real sign-off day.
CBOS still expects the gratuitous work form where that pathway applies. Open it before tools come out, not after the favour is a memory.

A vague association does not explain why the work is unpaid. Write family, friend, neighbour, or the charity or community link in plain words.
Describing the work as plumbing or gas work only.
That claims more scope than the job and tells CBOS nothing useful. Name the fixture, system, or repair and keep it matched to the unpaid work.
Guessing the insurance answer.
Yes or No is a declaration against the form's liability wording. Confirm the certificate of currency, or have the owner confirm their cover, before you select.
Using an old licence number or stale certifier details.
Saved certifier details go stale. Check name, date of birth, licence number, and contacts against the current licence before download.
Leaving owner sign-off until after the job.
Once everyone has gone home, chasing a signature and a reviewed description is hard. Capture sign-off when the owner has read the form.
Using the work site as the certifier home address.
Home address identifies you; work site identifies the job. Mixing them produces a PDF that looks complete and is wrong in two places.
Lodge the finished PDF with CBOS for assessment. The form directs you to post it to Consumer, Building and Occupational Services, PO Box 56, Rosny Park TAS 7018, or email cbos.info@justice.tas.gov.au. There are no fees to lodge this form with CBOS under the wording on the form.
Name the file so the site and date are obvious. For example Gratuitous work - Battery Point - 15 Jun 2026.pdf.
Keep proof of lodgement. Save the sent email or postage note with the PDF. Gratuitous work often has no invoice to anchor the job file.
File it with owner notes, photos, and insurance checks. The PDF alone does not show what you photographed or what cover you relied on.
Do not treat download as lodgement. Tradie Forms does not send the form to CBOS for you.
Check the current CBOS requirements for your job before you rely on the completed certificate.
