How to complete the WorkSafe Victoria asbestos removal notification (FOR567) in Tradie Forms.

This form notifies WorkSafe Victoria before licensed asbestos removal: who is removing it, what and how much, when, and where. The licensed removalist completes a new notice or an amendment, then downloads the official FOR567 PDF to email or fax the WorkSafe regional office closest to the site. WorkSafe Victoria says you need at least 24 hours notice for licensed non-friable removal of 10 m² or less, at least 5 days for all other licensed jobs, or within 24 hours of starting when an unexpected situation applies.
This is the current WorkSafe Victoria Asbestos Removal Notification and Amendment Form. Tradie Forms follows the official section order and maps your answers onto the official PDF layout.
Almost every late notice comes from filling against the booking sheet instead of the licence, the site assessment, and the real start date. Have these in front of you.
Current asbestos removal licence
Licence number, expiry date, and ABN or ACN for the business doing the removal.
Site assessment and material scope
Friable and non-friable types, quantities (m³ and m²), enclosure method if friable, and exact work areas.
Crew list
Supervisor names and mobiles that will be answered during the job, plus how many workers will remove asbestos.
Schedule and notice decision
Start and end dates, start time, estimated days, and whether this is new, an amendment, or unexpected-situation work.
Workplace and removal address
Client who commissioned the work, Victorian site address, and any telecom pit or pipe locations.
Existing WorkSafe reference when amending
The notification number from the original lodged notice if dates, scope, or site details change.
Notification type, your name and email, licence number and expiry, asbestos type (at least one of friable or non-friable), removal start date, street address and postcode, declaration tick, completer name, and declaration date gate the download. Friable and non-friable detail sections only appear after you tick the matching type.
Tells WorkSafe whether this is a fresh notice or a change to one already lodged. The same FOR567 is used for both.
A standing notice restates the notice periods: at least 24 hours before removing 10 m² or less of non-friable asbestos, at least 5 days before all other jobs, or within 24 hours when an unexpected situation applies.
New Notification for a job WorkSafe has not been told about. Amend Existing Notification when dates, quantity, location, crew, or other details on a lodged notice have changed. Do not open a second new notice for a schedule slip on the same job and location when an amendment is the right path.

Licence and business details for the removalist doing the work. These values need to match the current licence record, not a quote or old job file.
From the current asbestos removal licence, for example AR123456. Use the number for the business removing material on this site. If you are on site without the card, the licence holder or office can confirm it.
Expiry on that same licence. The field does not accept a date before today, so a lapsed licence cannot be entered as current.
ABN or ACN of the licensed business. Use ABN lookup when you have it. Match the entity on the licence, not a related trading name.

Selects friable, non-friable, or both. The next sections only appear for the types you tick. You need the licence class that covers the material: Class A for friable, Class B for non-friable as a minimum.
Download is blocked until at least one of Friable or Non-Friable is ticked, even though neither box is marked required on its own.
Tick Friable, Non-Friable, or both. Match the site assessment and licence scope. Tick both when the same job and location include both materials.
Which friable materials are being removed. Appears only when Friable is ticked under Asbestos Type.
Options include asbestos-backed vinyl sheeting, contaminated dust or soil, fuses in switchboards, insulation, millboard, non-friable asbestos that has become friable, pipe lagging, sprayed limpet, loose fill asbestos insulation, and Other.
Tick every friable material type on this job. Take the list from the register, survey, or site assessment, not from a similar job.

How friable removal is controlled. Appears only when Friable is ticked.
Options are Full Enclosure, Partial Enclosure, Glove Bag, Wrap and Cut, Other, and None.
Tick the method or methods that apply. Match the removal plan and controls you will use on site.
Enclosure methods outside the listed options, short but clear enough for WorkSafe.
Why no enclosure is required for this friable work. Leave blank when an enclosure method is selected.

Which non-friable materials are being removed. Appears only when Non-Friable is ticked under Asbestos Type.
Options include AC pipe, AC roof, AC sheet, contaminated soil, telecommunications pits and pipes, vinyl floor or wall tile, zelemite boards, and Other.
Tick every non-friable material type that applies. Match the register, survey, or site notes for this address.
Materials outside the listed options, in plain site language.
Estimated total non-friable area. Appears only when Non-Friable is ticked. A form notice reminds you that more than 10 m² of non-friable material needs at least 5 days notice.
The square metre figure decides whether the job sits under the 24-hour small-quantity rule or the standard five-day notice. Estimate it honestly before you lock a start date.
Estimated total in square metres, for example 8. Use the assessment total for this job, not a rounded-down figure chosen to fit a shorter notice period.
Supervisors for the removal work, each with a mobile answered during the job.
One row is one supervisor. Add up to five. Leave unused rows out rather than adding blanks. Each row uses Supervisor Name and Mobile Number.
Name and mobile for each supervisor. Prefer numbers that stay with that person for the job. If supervisors change after you notify, amend the notice before relying on the old details.
How many people will remove asbestos on this job. A form notice says that if worker training or experience has changed from what you previously told WorkSafe, update your asbestos licence at myworksafe.vic.gov.au.
Headcount of workers who will remove asbestos, for example 4. Match the crew for this job, not the full company headcount.
When removal starts, how long it runs, and whether the work is an unexpected situation. The start date is what the notice period is measured against.
First day of licensed removal on this notice. Does not accept a date before today. If the start slips after you lodge, use an amendment rather than working on a date WorkSafe was never told about.
Last planned day of removal. Keep it consistent with estimated days.
Planned start time on the start date, for example 07:00.
Expected days. Count calendar days including weekends: Friday to Monday is 4 days, not 2. The app helper states the same rule.
A sudden event that may expose people to airborne asbestos, an unexpected breakdown of an essential service needing immediate rectification, or None of the Above. Unexpected-situation work is notified within 24 hours of starting rather than in advance.

Emergency
The kind of place where removal will happen. Tick the site type, not your business type.
Options include Domestic Home, Public Place (school, hospital, shops), Office or Factory, Demolition Site, Commercial Roof Work or Removal, Utilities, Next to Public Place, and Other.
Tick every type that fits the removal site. A domestic home next to a public footpath may need more than one tick when both apply.
Short, specific description when Other is selected, for example a school building, retail tenancy, or industrial shed.
Who commissioned the asbestos removal. Use the client on the engagement, not only a gate contact who met the crew on the day.
Import from ServiceM8, Fergus, or Xero where supported. After import, check the contact is the workplace or engagement contact, not a billing-only Xero address.
Who engaged the licensed removal, for example Harbour Holdings Pty Ltd. Match the quote and job booking.
Person WorkSafe or the crew can call about access, for example Chris Nguyen.
Phone for that workplace contact during the removal window.

Victorian site name and street address. The address decides which WorkSafe regional office gets the PDF, so use the removal location, not head office or billing.
Use address search (Victoria only). It fills street line, suburb, state, and postcode. Check unit, level, lot, or tenancy after search. Import from ServiceM8, Fergus, or Xero where supported, then confirm it is the removal site. State defaults to VIC. The official PDF has no separate state widget. Suburb is required for download and is filled by address search.
Short site or job name, for example 42 Industrial Drive Refurbishment. Helps tell multiple notices apart when the street alone is not enough.

Email or fax the completed PDF to the WorkSafe regional office closest to the removal site. The form lists the regional office contacts. A separate notification is required for each job and location.

Five days is the default for friable work and for non-friable over 10 m². If unsure, notify five days out.
Underestimating non-friable quantity to fit the shorter notice.
The square metre figure sets the deadline. Calling a larger job 10 m² or less does not change what WorkSafe expects once the real scope is on site.
