## Before the work: Forms 1, 2 and 12 [#before-the-work-forms-1-2-and-12]

**Form 1 - permit work application.** Business Queensland says permit work needs a permit before it starts, and that the work is inspected against the relevant plumbing laws, the permit terms and the plans lodged with the application. The risk here is rarely the form number - it is stale job details, a missing lot and plan, or a fixture count that changed after a builder meeting. The supporting documents guidance says extra documents may be needed depending on the proposed work and building class. **Form 2 - amend a permit or extend time.** Use it when the real job has moved away from the approved job: fixtures changed, the treatment plant changed, a site condition forced a different run, or more time is needed. The Queensland guidance says an extension should be applied for at least 10 business days before the permit term ends, and that an amended permit needs to be approved and issued before the extra work starts. **PDR Form 12 - compliance statement for specialised work.** This is the plumbing and drainage Form 12, not the building one. The official PDF asks for the description of work, the basis of the statement, reference documentation and suitably qualified person details, so the statement can be attached to permit paperwork and reviewed. ## During the work: Forms 3 and 5 [#during-the-work-forms-3-and-5]

**Form 3 - covered work declaration.** Business Queensland describes this as an alternative in some cases where local government may accept a declaration or report instead of completing an inspection. The guidance says it can be used where work is covered before an inspector has inspected it and the local government has allowed the form to be used to certify the work is compliant. **Form 5 - testing or commissioning report.** It records what was tested or commissioned, and it often sits beside permit or inspection paperwork because the result explains whether the job can move forward. Fill it while gauges, readings and equipment details are still in front of you, not from a notebook on Friday. ## Recurring and device work: Forms 9 and 11 [#recurring-and-device-work-forms-9-and-11]

**Form 9 - backflow registration and report.** The registration and report on inspection and testing of testable backflow prevention devices. The device, location, owner, property, readings and tester details all need to match the asset and the council record. The official PDF says copies must be submitted to the relevant local government and the owner of the premises within 10 business days after inspecting or testing the device. **Form 11 - treatment plant service report.** A service report for an on-site sewerage or greywater treatment plant. The official PDF says it must be submitted to local government with a copy provided to the owner within 10 business days after servicing the facility. It covers plant details, system status, land application area, service tests, annual tests and owner details. ## At completion: Forms 14 and 19 [#at-completion-forms-14-and-19]

**Form 14 - compliance declaration.** Tied to completed work and a relevant action notice. The official PDF asks for permit details, action notice details, a description of the work performed, the completion date, the responsible person, contractor details where needed, and the declaration. The declaration has to match the action notice and the work actually done. **Form 19 - final inspection certificate.** This one is not a plumber's declaration. Business Queensland says Form 19 is used by the local government or a public sector entity, that it certifies the permit work is compliant, operational and fit for use, and that it can be issued when all work under the permit, or a distinct part of the work, is complete. Keep the finished PDF with the evidence that explains it: plans, fixture schedules, test sheets, commissioning notes, backflow asset details, treatment plant service notes, photos, council emails, owner approvals and the job reference.
