The Housing Industry Site Safety Pack, or HISSP, is a set of ten work health and safety templates that SafeWork NSW gives away free. It is built for small residential construction subcontractors, up to about ten workers, who need to show a principal contractor and the regulator that they have a real safety system rather than a folder of good intentions.

Form 08 is the electrical test and tag register in that pack. It sits alongside the risk assessment, the safe work method statement, the tool box talk record, the training register and the incident register. On a house site the builder or principal contractor often collects these from every trade before work starts, so a missing or blank register is the kind of thing that holds up your start on site.

The register does one job. It lists every item of electrical equipment that has been inspected and tested, records the result, and sets the date the item is due again. The tags on the tools are the quick visual check. The register is the record behind them.

## Who has to test, and who keeps the register [#who-has-to-test-and-who-keeps-the-register]

Two roles show up on the form, and it helps to keep them separate.

The register is kept by the business running the work on site, usually the principal contractor for the project. The testing itself is carried out by a licensed electrical contractor or a trained test and tag operator. The form has a field for the business keeping the register, a field for the electrical business doing the test and tag, and a field for the tester. If your company does its own testing, the business name simply repeats.

The person doing the work has to be a competent person. Under AS/NZS 3760, the in-service inspection and testing standard, a competent person has the knowledge and skill from training, qualification or experience, plus the correct test equipment, to inspect and test the item and read the result properly. It is not a job for whoever is free that afternoon. The Work Health and Safety Regulation 2017 (NSW) backs this up: electrical equipment used in conditions that could damage it must be regularly inspected and tested by a competent person, and a record kept.

## Business details at the top [#business-details-at-the-top]

The first section of Form 08 is the header that ties the register to a site and a business.

**Business name and address.** The site business or principal contractor keeping the register for the project, with the NSW site address. This is what links the register to the job.

**Electrical business name.** The licensed electrical contractor performing the test and tag. If the same company keeps the register and does its own testing, put the business name in again.

**Tester's name.** The person who actually carried out the inspection and test. This is the name that has to stand behind the results in the rows below.

Fill this once for the site and it frames every equipment row that follows. Getting the address and business names right here is what lets a builder or an inspector match the register to the right job and the right contractor.

## The equipment rows [#the-equipment-rows]

The body of the register is a row for each item tested. This is where most of the detail lives, and where a rushed entry causes trouble later. Each row on Form 08 captures:

**Description, manufacturer and serial number.** What the item is, who made it and its serial or asset number. An extension lead, a portable RCD, a drill or a site light each gets its own row. The serial number is what lets you find the same item next quarter rather than guessing which lead was which.

**Date of inspection and result.** When you tested it and whether it passed or failed. A pass and a fail are both real outcomes worth recording, because a failed item that is removed from service is part of the story the register tells.

**Action and signature.** What happened to the item, whether that was a tag applied, removed from service, repaired and retested, or quarantined pending repair, and the tester's signature against the row. A failed lead with no action recorded looks like an oversight.

**Certificate number and next inspection date.** A reference for the test certificate and the date the item is due again. On a construction site the next date is usually three months out, and it should match the date printed on the physical tag.

The register allows up to twelve items in one pass, which covers a typical round of leads, tools and portable RCDs on a house site. The point is that every tag on a tool has a matching row here, with the same next test date, so the visual tag and the written record never disagree.

## How often testing is due [#how-often-testing-is-due]

Frequency is the fact people most often get wrong, because the interval depends on the environment, not just the tool.

A construction, demolition or mining site is a harsh place for electrical gear. Leads get dragged across rubble, tools get dropped and everything lives in dust and weather. Because of that, portable electrical equipment on these sites is inspected, tested and tagged every three months. That interval comes from AS/NZS 3012, the standard for electrical installations on construction and demolition sites, read together with AS/NZS 3760 for the in-service testing itself. The reference section on Form 08 spells this out so nobody sets the next date twelve months away by habit.

Other workplaces are gentler on equipment and are usually tested every six or twelve months, again depending on the environment and the assessed risk. If you also work in offices, factories or hire environments, do not carry the softer interval onto a house site. The site sets the frequency.

Portable RCDs are their own case. On construction sites they are subject to an operating time and push button test at set intervals as well, so a portable safety switch on a lead is checked more often than the lead alone. ## What goes on the tag, and why the register matters [#what-goes-on-the-tag-and-why-the-register-matters]

AS/NZS 3760 sets what a durable tag has to show: the test date, the next test date, and details identifying the item and the person who tested it. On site the tag is the fast check. An inspector or a leading hand can pick up a lead, read the tag, and see at a glance whether it is in date.

The register is the layer underneath. If a tag is torn off, faded or missing, the written record on Form 08 is what proves the item was tested and when. If SafeWork NSW asks to see your system, they will want the tags on the tools and the register that backs them. A stack of tagged leads with no register, or a register that does not match the tags, is a weaker position than either on its own.

That is the real reason to keep the register current rather than reconstruct it later. Written up at the item, with the serial number and result in front of you, the register and the tags line up. Written up from memory in the ute a week later, they drift.

## Common test and tag register mistakes [#common-test-and-tag-register-mistakes]

These are the ones that turn a tested tool into a compliance gap.

**Setting the wrong next date.** A twelve month interval on a construction site is the classic error. On a house site the default is three months. Set the next date to match the tag and the site.

**Rows that do not match the tags.** If the register lists ten items and there are fourteen tagged tools on site, the record is incomplete. Every tag needs a row.

**Blank results or actions.** A row with no pass or fail, or a failed item with no action recorded, is unfinished. Fill the result and, where an item fails, what you did about it.

**Missing serial numbers.** Without a serial or asset number you cannot reliably match the same item next quarter, and two identical leads become impossible to tell apart in the record.

Most of these come back to the same habit. The register is written up away from the equipment, from memory, after the tags are already on. The fix is to complete it at the item while the details are in front of you.
